Scheduling an Appointment
Establishing care with a gastroenterologist ensures timely access to colonoscopy, endoscopy, and advanced digestive
care when you need it. New and existing patients can request appointments online or by phone.
If this is a medical emergency, call 911 or go to the nearest emergency department.

Being Seen In the Clinic ?
Remember to bring…
- Completed medical history form
- Current medication list form including dosage and frequency, including over-the-counter and herbal supplements
- Insurance card(s), co-pay and picture ID
- Copies of recent tests, imaging reports (x-rays, CAT Scans), past procedure reports or anything you want your provider to review during you’re appointment
- Arrive 15 minutes early for check-in; We are located in building “C” on the Salem Hospital campus. Allow plenty of time for parking and finding our clinic.
Having a Procedure?
Remember to…
- Bring your photo ID and insurance card
- Arrive 15 minutes early for check-in
- Review your preparation instructions
- Arrange a responsible adult who can wait during your procedure (approx. 2hrs)
and drive you home - Wear loose comfortable clothing
- Bring current medication and allergy list
- Leave valuables at home (except insurance card(s) and deposit; if required)
Appointment Types
Office Visits
Consultations and follow-ups for symptoms such as reflux, abdominal pain, bleeding, bowel changes, IBD, liver disease,
and other digestive conditions.
Colonoscopy & Endoscopy
Screening and diagnostic procedures for colon cancer prevention and evaluation of upper GI symptoms. Some procedures
require a referral and specific preparation.
Telehealth (Selected Visits)
Certain follow-ups may be available by video. Call to confirm whether your visit is appropriate for telehealth.

- Inpatient hospital care (outside of endoscopy services)

Insurance Information
Salem Gastro participates with many commercial plans and Medicare/Medicare Advantage plans. Coverage varies by plan and product—please call to confirm your benefits and any referral requirements.
We do not accept “out of network” insurance plans.
Please call 503-399-7520 option 7 for more information or to verify your insurance plan’s status.

Patient Portal FAQs
- How do I sign up? When you register as a patient you should receive an invite. If you have not received an invite, call our office and request one from Medical Records (option 5).
- Can I view procedure prep? Yes—prep instructions and appointment details are available in the portal.
- How do I request refills? Send a secure message through the portal or call the clinic.
- How do I get lab or pathology results? Results are posted in the portal once reviewed; your provider may also call you.
- Can I upload forms? Yes—use the portal’s document upload or bring printed forms to your visit
